You do not need to know your diagnosis before booking. Choose the option that best matches your situation, and our team will help guide you from there.
Pain, stiffness, weakness, or limited movement
Start here if you have pain or movement problems but are not sure what condition you have. Your evaluation may look at where you feel symptoms, how your body moves, and what Level of Care may fit your stage.
Car accident / No-Fault
Start here if your pain began after a car accident, even if symptoms were delayed. Some patients notice pain right away. Others feel worse hours or days later. Your evaluation helps determine your starting point and what kind of care may be appropriate.
Work injury / Workers Comp
Start here if your pain began at work, during lifting, standing, driving, repetitive tasks, a fall, or another job related accident. Workers’ Comp cases may involve claim information, employer details, medical referral, and authorization steps. Our team can help you understand what to bring.

Sports injuries may involve pain, weakness, instability, stiffness, poor control, or difficulty trusting the injured area. Your evaluation helps determine whether your body needs protection, gentle movement, strengthening, or higher level return-to-sport progression.
Post-surgical rehab
Start here if your surgeon, doctor, or medical provider recommended physical therapy after surgery. Postsurgical care often depends on your surgery type, precautions, tissue healing, pain, swelling, motion, strength, and surgeon instructions. Your physical therapy plan should match your stage and restrictions.
Balance, dizziness, walking, or fall concern
Start here if you are worried about falls, dizziness, gait, neuropathy-type balance problems, vertigo type symptoms, or walking confidence. Your evaluation may look at balance, strength, walking tolerance, safety, and movement control
